SWBHIPA Diabetes Project Training
When: Monday Feb. 24, 2020 5:30 p.m – 7:30 p.m.
Where: 3200 Carlisle NE basement classroom
What: 2nd Training for the True Health NM Diabetes Project; 2 CEU’s will be offered
During this training, SWBHIPA member Dana Paine will be talking about the medical, nutritional and physiological issues that can occur with diabetes. This is based on his 15 years of experience dealing with this population. Additionally, SWBHIPA member Cynthia Sontag will address the multidimensional issues that need to be looked at in terms of treatment and the prevalence of depression in this population. A light supper will be provided.
How the coordinated efforts of True Health NM and SWBHIPA referrals work:
All True Health members with diabetes are automatically sent to their Case Management team; part of what the True Health case managers do is administer the Diabetes Distress Scale as a screening tool and offer behavioral health support in addition to any needed medical/nutritional resources. If their member is responsive to talking with a behavioral health provider, the case manager sends a referral email out to an identified group of SWBHIPA providers who are interested in these referrals and who have completed this training. The first training was done in 2018. True Health has been pleased with their ability to directly link a member with diabetes to a mental health practitioner and has asked the SWBHIPA Liaison Committee to identify additional providers interested in working with this population.
